Eligibility for Admission: A candidate desired to join four year programme leading to the B.Sc Optometry degree ® Should have passed the two’ year Pre University examination conducted by Department of Pre University Education, Kamataka State with English as one of the subjects and Physics, Chemistry Biology/Maths as optional subjects. The candidate shall have passes the subjects of English. Physics, Chemistry, Biology/Maths individually also.

OR ® Shall have passed any other examination conducted Boards/Councils/Intermediate examination established by State Governments/Central Government and recognized as equivalent to two year Pre University examination by the Rajiv Gandhi University of Health Indian Universities (AIU) with English as one of the subjects and Physics, Chemistry and Biology as optional subjects and the candidate shall have passed subjects of Biology/Maths individually also. ®Shall have passed intermediate examination in Science of an Indian University/Board/Council or other recognized examining bodies with Physics, Chemistry and Biology which shall include a practical test in these subjects and also English as compulsory subject. The candidate shall have passed subjects of English. Physics, Chemistry, Biology/Maths individually also.

1) Shall have passed first year of the three year degree course of a recognized University with Physics, Chemistry and Biology including a practical test in these subjects provided the examination is an ‘University Examination’ provided that the candidate shall have passed subjects of English. Physics, Chemistry, Biology/Maths individually in the pre university or other examinations mentioned in the clauses above.

2) Shall have passed B.Sc Examination of an Indian University, provided that he/she has passed the B.Sc examination with not less than two of the following subjects: Physics, Chemistry, Biology (Botany, Zoology) provided the candidate has passed subjects of English, Physics, Chemistry, Biology/Maths individually in the qualifying examinations mentioned in clauses a. b. and c.

Discipline and General Conduct
- Students are required to maintain very high standards of discipline all acts of indiscipline by the students will be dealt with severaly through administrative means.
- The practice of ragging is illegal and is strictly prohibited. Offenders are liable to be prosecuted as per law.

- Inability to meet the minimum prescribed attendance norms of a university and poor academic performance will render the student liable to be debarred from appearing for exams.
- Students are required to dress decently as per accepted social practice and are expected to be well manned in their interaction with staff members and other students
- Identity card must be carried by the student at all times and must be presented for inspections on demand by the college authorities.
- The students are permitted to meet only authorized visitors and guardians. Visitors are allowed only during prescribed visiting hours. No unauthorized visitors should be entertained by the student in the college / hostel premises.
- Breach for discipline and failure to abide by the rules and regulations of the institute and hostel will render the student liable for disciplinary action,
- If any incidents of ragging comes to the notice of the authority, the concerned student shall be given liberty to explain and if his explanation is not found satisfactory, the authority would expel him from the institution.
 
Documents to be submitted at the time of interview / admission
- Original marks card / pass certificate or equivalent examination with three photocopies, duly attested.
- SSLC pass certificate or any other documents mentioning the date of birth in original with three photocopies, duly attested
- Eligility certificate ( in case of non-Karnataka students ). Application form for issue of the same can be obtained from our office on request at the time of admission. The application should be duly filled in the copies of documents attached and compete in all respects. The candidates are requested to obtain the same form Rajiv Gandhi University of Health Sciences, Bangalore before admission is made.
- Transfer certificate issued by the institution last studied in original with three photocopies duly attested.
- NRI category candidates are required to produce the attested copy of their passport for obtaining eligibility certificate from the University.
- Certificate of conduct / character from the head of the institution last attended original copy.
- Photographs : passport size 5 copies, stamp size 5 copies and in the case of NRI candidates, passport size 8 copies and stamp size 8 copies ( in addition to the one affixed on the application form )
- Medical fitness certificate including blood group certificate
- Candidates belonging to SC / ST category should produce a caste / income certificate issued by the competent authority with an affidavit on Rs. 20/- stamp paper signed by the parent or guardian ( in case the parents are not alive ) duly attested by a magistrate or a public notary. In case of candidates whose parents are not alive and who produce the affidavit from the guardian, death certifies of both parents should be attached.
- In case of admissions under the Thulu linguistic minority category certificate issued by the Tahsildar as proof of mother tongue Tulu
- A copy of each of these documents duly attested are to be attached to e application form and the original documents + 2 copies each attested should be provided at the time of interview / admission
